Addendum to the Probate Order Special Conditions | This would be executed in cases involving a probation revocation |
Admittance of Positive Screen | This document would be completed and submitted to the Coordinator if the participant believes they are going to fail a drug screen. |
Consent for Assessment | This document allows the Coordinator to perform an assessment on a defendant that has been referred to the program. |
Court Contract | Each Participant must complete a contract before being permitted to enter the program. |
Court Information | This document provides basic information on the requirements and expectations of the Treatment Court |
Criminal History Consent Form | This document allows the court to receive a current criminal history |
Drug Sheet | This sheet contains a list of medications which would be prohibited in this program absent specific approval prior to use |
Incidental Exposure Contract | This document is executed following acceptance into the program |
Intake Form | This form is completed and provided to the Court Coordinator to give more detail on the program participant. |
Medical Provider Advisory | This document should be provided by program participants to any treating physicians |
Medical Release Form | This document would be completed by a participant to allow the Coordinator to receive medical information. |
Participant Handbook | The Participant Handbook sets out the requirements for the program. |
Participant Request Form | This form is used by participants to make a request to the Team for consideration. |

Sentencing Consent Order | This document would be executed when entering a plea to a new charge |
Testing Compliance | This document is executed after acceptance into the program |
