Bid Synopsis
***Proposal due date extended to 4/8/11 @ 4PM.***Cherokee County, located 35 miles north of the City of Atlanta, Georgia, is now accepting Proposals for Wellness Program and Related Services, including team submittals or separate proposals for health and welfare advisory and brokerage services. The requirements for submitting a Proposal are stated in the attached Request for Proposals (the "RFP"). Please review them carefully. Service Providers who intend to submit a Proposal shall submit the RFP Acknowledgement Form no later than March 15, 2011, to the email address listed in Section 2.2. One electronic copy of the Proposal should be sent via email and three (3) hard copies of your Proposal must be submitted via mail. All Proposals are due to: Greg Dodgen, Director of Procurement & Risk Management, Cherokee County, 1130 Bluffs Parkway, Canton, Georgia 30114, gdodgen@cherokeega.com no later than April 1, 2011 at 4PM.
