Wellness Program
Cherokee County is seeking to make a significant impact on future medical claims by addressing the health of its population. The idea is to impact Plan costs in a positive way by reducing demand for services via a more healthy population. In order to impact the health of the plan members Cherokee County has entered into an agreement with Communit-Y Health Network (CHN) that offers a program which incorporates intensive face-to-face coaching for high risk employees. High risk is defined as having either a single critical health risk factor or a combination of health risk factors that could lead to high cost medical care.
 
In order to access employee health, Cherokee County will offer biometric screening which will measure risk factors such as blood sugar, blood pressure, cholesterol and body mass index (a measure of weight to height). Additionally, participating employees will complete a health risk assessment (HRA). Based on the results of the biometric testing and HRA, employees will be segregated into one of three risk categories – high, medium, low. 
 
Participation in the wellness program and the testing, coaching and wellness classes that are offered as part of the program is voluntary. Employees electing to participate in the program will be incentivized through a reduction in premium contribution rates.

http://www.myfitnesspal.com/ My Fitness Pal is a free online tool that allows you to set weight goals.  Once your goal is established, the program then calculates the amount of calories needed to hit your mark.  Participants enter their daily food choices and phyical activities as a way to journal their progress toward reaching the goal.  Participants must create a log-in which allows them "join" various communities and share information with others.
