Our Case Managers assess the needs of each client referred to the Home Delivered Meals (Meals-On-Wheels) and/or Homemaker Services programs. Through consultation with the client, they then design an individual plan and coordinate services through Cherokee County Senior Services and other community agencies as needed. Our Case Managers then work with each client on an on-going basis to ensure that services are being provided and their needs are being met.

Case Managers assist with:
Housing assistance, including help with assisted living and nursing homes
Financial assistance
Legal information and referrals
Medical equipment
Meals-On-Wheels
And many others
