How can I find a provider in the NVA network?
To get the provider list for the plan, log in to the NVA website http://www.e-nva.com/ or the
NVA app and click on the Find a Provider link. You will need to enter a zip code and up to a 30-
mile radius for your provider search.

Do I receive an ID card? Or need one to go to the eye doctor?
It is important to know you do not need an ID card to access your vision benefits. Simply select a
participating provider, call for an appointment and them that NVA is your benefit administrator.

If you are needing replacement cards, you can log in to the NVA system http://www.e-nva.com/
and print them from the website or log in with the app and flash your ID card at the time of the
service. Of course, you can always call 24/7/365 the customer service center at 800-672-7723.
