Case Management

The Case Managers assess the needs of each client referred to the Home Delivered Meals (Meals-On-Wheels) and/or Homemaker Services programs. Through consultation with the client, the Case Manager designs an individual service plan and coordinates services through Cherokee County Senior Services and other community agencies as needed. Our Case Managers then work with each client on an on-going basis to ensure that services are being provided and make sure the client's needs are being met. The Case Managers are very dedicated to the senior population and go above and beyond to make sure the client is getting what he or she needs.

To be eligible to receive case management services, a client must be sixty years of age or older and reside in Cherokee County.
